MEDIA RELEASE FORM

Media Release Agreement

By signing this form, I grant Anthony B Media LLC and its representatives, clients, and partners permission to photograph, film, and record me and/or my property during the project listed above.

I authorize Anthony B Media LLC to use my name, image, likeness, voice, and appearance in photo, video, and audio content for business purposes, including:

• Marketing and advertising

• Social media (Instagram, Facebook, LinkedIn, YouTube, TikTok, etc.)

• Website, portfolio, and promotional materials

• Client deliverables and presentations

• Advertising campaigns and brand storytelling

• Editorial and educational content

• Print and digital media in any format, now or in the future

If I participate in an interview, I understand that my responses may be edited, shortened, or combined with other content to create a final production. Anthony B Media will use reasonable judgment and will not intentionally misrepresent my statements.

I understand that Anthony B Media LLC retains ownership of all original media created. Any final deliverables and usage rights provided to me will be outlined separately in my project agreement.

I waive any right to compensation, royalties, or approval of the final content unless otherwise agreed upon in writing.

I release Anthony B Media LLC, its owners, employees, contractors, and clients from any claims related to the use of this content, including claims involving privacy, publicity rights, or copyright.

This agreement is governed by the laws of the State of Michigan.
